The Online Monitoring mobile app was created for the Epidemiologists of the Southern Nevada Health District to help people self report during health-related, epidemic or pandemic cases such Ebola, Measles, or something new.

It was designed for emergency cases so that the administrators can dynamically create forms for speedy reporting.

Only the Southern Nevada Health District can instruct you to download the app.

In addition to developing the mobile app and integrating the API, I also designed the loading screen and the app icon.
